    Últimas investigaciones sobre el cannabis: cinco nuevos estudios para conocer

    En las últimas semanas, se ha publicado una investigación prometedora sobre el potencial del cannabis como herramienta para el sueño, la recuperación muscular, la esclerosis múltiple (EM) y la enfermedad inflamatoria intestinal (EII).

    Con un número récord de trabajos de investigación publicados sobre el cannabis y su potencial terapéutico en 2022, puede ser difícil mantenerse al día. Si bien los reguladores exigen más evidencia científica, se están realizando estudios en todo el mundo que investigan su papel en el manejo de los síntomas de una amplia gama de afecciones.

    Recientemente se han publicado una serie de estudios, incluidos varios ensayos controlados aleatorios (ECA) que se consideran el «estándar de oro» de la ciencia, que brindan nuevos conocimientos sobre sus efectos en la EM, la EII y el insomnio. Mientras que el CBD también se muestra prometedor como una herramienta para promover la recuperación muscular y mejorar la memoria en CUD.

    El cannabis medicinal mejora el sueño en adultos con insomnio

    Un estudio doble ciego aleatorizado controlado con placebo encontró que el aceite de cannabis medicinal mejoró tanto el tiempo como la calidad del sueño en un grupo de pacientes con insomnio autoinformado.

    Participaron un total de 29 participantes y fueron asignados al azar para recibir un placebo o un aceite activo que contenía 10 mg/ml de tetrahidrocannabinol (THC) y 15 mg/ml de cannabidiol (CBD) durante un período de dos semanas.

    Según el documento, los participantes mantuvieron un diario para evaluar la tolerabilidad, y la eficacia se midió mediante los niveles de melatonina en la saliva a medianoche, cuestionarios validados y el rastreador de sueño Fitbit. 

    Se descubrió que el aceite de cannabis mejora tanto el tiempo como la calidad del sueño. El sueño ligero aumentó en 21 minutos por noche en comparación con el placebo, y la calidad del sueño mejoró en general hasta en un 80 % en el grupo activo.

    Los autores concluyen que el aceite fue ‘bien tolerado’ y el 60 % de los participantes ya no se clasificaron como insomnes clínicos al final del período de prueba. 

    Cómo el CBD podría promover la recuperación muscular 

    El CBD ha ganado cada vez más popularidad entre los atletas desde que la Agencia Mundial Antidopaje ya no lo considera una sustancia prohibida. 

    Una revisión narrativa de la investigación existente encontró que el CBD tiene la «posibilidad de convertirse en un suplemento adecuado que puede mejorar la recuperación muscular». En estudios con animales, se demostró que el CBD es eficaz para aumentar la expresión de reguladores metabólicos en el músculo de ratones obesos, mientras que el tratamiento con CBD en roedores redujo la inflamación muscular después del ejercicio en un modelo de distrofia muscular.

    En humanos, hubo «algunos indicios» de que la suplementación con CBD mejoró la recuperación y el rendimiento muscular, aunque las dosis fueron «altamente variables», dicen los autores.

    El CBD puede mejorar la cognición en el trastorno por consumo de cannabis (CUD)

    CUD se define como el uso continuado de cannabis a pesar de las consecuencias adversas, que causa deterioros o angustia clínicamente significativos, y se estima que afecta a hasta 13 millones de personas en todo el mundo.

    En un ensayo clínico aleatorizado, los investigadores evaluaron si cuatro semanas de tratamiento con CBD afectaron la función cognitiva en personas con CUD. Setenta participantes fueron asignados aleatoriamente a un placebo, 400 mg u 800 mg de CBD. Los investigadores concluyeron que, si bien el CBD no influyó en la memoria verbal retrasada ni tuvo efectos cognitivos amplios, 800 mg de tratamiento diario pueden «mejorar la manipulación de la memoria de trabajo».

    El efecto del cannabis medicinal en la enfermedad inflamatoria intestinal (EII)

    Los datos del mundo real tomados del Registro de Cannabis Médico del Reino Unido brindan una idea del efecto de los medicamentos a base de cannabis (CBMP) en pacientes con EII. 

    Setenta y seis pacientes se incluyeron en la serie de casos, con resultados evaluados al mes y a los tres meses, a través de una serie de cuestionarios validados, incluido el Cuestionario de enfermedad inflamatoria intestinal breve (SIBDQ), el Trastorno de ansiedad generalizada-7 (GAD-7), Escala de calidad del sueño de un solo elemento (SQS) y puntuación del índice EQ-5D-5L.

    La puntuación inicial promedio del SIBDQ mejoró al mes ya los tres meses, mientras que las puntuaciones de calidad de vida, ansiedad y sueño también mejoraron a los tres meses. Un poco más del 20% de los pacientes informaron eventos adversos, siendo la mayoría de gravedad leve a moderada. 

    Los autores concluyeron que los consumidores anteriores de cannabis «informaron de una mayor mejora en comparación con las personas que nunca habían tomado cannabis».

    Esclerosis múltiple (EM) y uso de cannabis medicinal 

    Una revisión retrospectiva de las historias clínicas de 141 pacientes con EM tuvo como objetivo evaluar el efecto del cannabis medicinal en sus síntomas. 

    Según el artículo, los pacientes experimentaron una «gran mejoría de los síntomas de la EM» después del inicio del tratamiento con cannabis, siendo el alivio del dolor (72 %) y la espasticidad (48 %) y la mejora del sueño (40 %) los más comunes.

    También hubo una «reducción significativa» en el uso de opioides entre los pacientes. 

    Los autores concluyeron: “En muchos pacientes con EM, el MC fue bien tolerado, alivió el dolor y la espasticidad, mejoró el sueño y otros síntomas, y redujo el uso concomitante de analgésicos opioides. Se necesitan estudios prospectivos para investigar más a fondo el papel de MC en el tratamiento de pacientes con EM”.
